I think your brother should first stop measuring his health by unconventional ways.

He should avoid "trying to be fat", you know, eliminating his skinny-ness.
If you could check out how being overweight or over fat is, you will realize that measures such as body mass ratio, the relation of weight and height and age, are the best to evaluate his health position.

Anyway, feed him healthy balanced diets. I insist, let the diet be healthy. A pizza may be balanced but it is not healthy.

Let him start with exercise he is comfortable with without jumping his gun and doing adult workouts.

As teenagers go through a lot of changes, the body is constantly rebuilding itself. This therefore manifests in his health outlook.

Finally, avoid being overly self conscious. You lose, you gain, everything will be alright at the end of the day.
